NDIS Support Coordinators

Neurological Physiotherapy in the home and community

Active Edge Physio has over 3 years of community based experience working with adults living with neurological disabilities who receive NDIS funding. We are a trusted provider that pride ourselves on the quality and detail in our assessments, intervention and NDIS report writing particularly in the context of complex disabilities.

We are involved with NDIS participants throughout inner and outer Melbourne, working with Support Coordinators and Local Access Coordinators to help our clients achieve their goals.

Working with our participants to develop their independence

We work towards our participant’s goals as outlined in their NDIS plans, create service agreements and request a variety of Assistive Technology (AT) equipment.

Some examples of assistive technology that our physiotherapists prescribe include the Philips Respironics Cough Assist E70 Machine, Nippy Clearway, Non Invasive Ventilation, Power Wheelchairs, Ceiling Hoists and Standing Machines.

The hourly rate we charge for our services is set by the National Disability Insurance Agency (NDIA), the Australian government organisation that is responsible for managing the NDIS. NDIS Frequently Asked Questions

We have over 3 years experience working with NDIS participants. During that time we have found that seeing a participant’s full plan helps us to achieve better outcomes together.

Seeing the plan helps us to:

  • Understand your prioritised goals and the available supports
  • Discuss openly what we can achieve with the available resources
  • Support participants to engage other services so that you get the most out of your funding
  • Communicate clearly with the participant’s providers and support networks
  • Write accurate reports to help the participant get the funding they require in future NDIS plans
  • Request funding for areas of unmet need
  • Reduce hours spent asking the participant or NDIS Support Coordinator for information about the plan

We understand that it is each participant’s choice as to whether they choose to share their plan. We respect their right to choose who they share their plan with. Please be assured that we do not access any funding outside the services that the participant have engaged us to provide.

We understand if the participant does not want to share all sections of their plan. There are some areas we require in order to process a referral. This includes:

  • The participant’s current address
  • Plan dates
  • Plan goals
  • A snapshot of the Capacity Building Budget

Active Edge Physio invoice travel to appointments in a participant’s home or within the community, and from the last participant of the day in 5 minute increments at our standard hourly rate. We bill a maximum of 30 minutes to the participant, and 30 minutes from the participant at the rates stipulated in the NDIS Price Guide.

Where a therapist travels to provide services to more than one participant in the same geographical area, the therapist will apportion travel time between participants equally.

Travel charges can vary week to week as it depends on where a therapist’s previous participant is, how many participants are booked, whether there are cancellations, and a number of other factors. Our team prioritises scheduling appointments to minimise travel costs.

We will organise an appointment for your participant in the first instance. Following our initial assessment, our recommendations and a service agreement will be created and circulated to the participant, support coordinator and plan manager. Our service agreement outlines the physiotherapy support to be provided under the participant’s NDIS plan and how much funding has been allocated.
